| Through the Representative Payee Program,
Opportunities is assigned by the Social Security Administration to assist
individuals with financial management who receive Social Security benefits,
Supplemental Security Income, and/or Social Security Disability. These funds
are administered by Opportunities on behalf of families that need assistance
with their finances. Opportunities distributes these monies for the family's
daily living needs, as well as managing saving accounts and burial funds. |

In 2007, 23 individuals
received Representative Payee services through Opportunties. |
| Program Highlights: |
| 1. |
The Consumer must receive Social Security benefits and may be
referred by other agencies, family members or self. |
| 2. |
This program may or may not be voluntary.

| 3. |
Opportunities is responsible for paying all personal and
household bills for consumer. As a footnote, grocery accounts can be
set up with local grocery stores. |
| 4. |
After all monthly and other bills have been paid the
Consumer receives the balance in a weekly personal check.
|
| 5. |
The Representative Payee Program
also involves intensive case management. Consumers are contacted about once a month,
and more frequently if needed.
